Family Health Teams : InfoKit |

 Since they were introduced in 2004, communities across Ontario have embraced the Family Health Team concept and have responded with overwhelming enthusiasm to past calls for applications. Since that time, 150 Family Health Teams have been established across the province in a variety of settings and communities. The government has identified its commitment to add 50 new Family Health Teams as one of its key initiatives to improve access to comprehensive family health care. Further information on the application process will be communicated here. Please check back often for any updates on the application process for the 50 new FHTs. |
