Questions & Answers on the
Application for Mobility Devices Form
The following series of questions (Q) and answers (A) are provided to support ADP Registered Authorizers and ADP Registered Vendors as they confirm an individual’s eligibility for funding assistance, complete and submit an application form, or dispense and invoice for ADP approved mobility devices. The Administration Manual dated March 2007 contains all the policies and procedures related to ADP funding of mobility devices.
