I no longer wish to be enrolled with my current health care provider. How do I cancel my enrolment with my current provider so that I can register with the Health Care Connect program?

A patient can contact their current health care provider indicating that they wish to end enrolment OR telephone ServiceOntario at 1-866-532-3161.


How soon will I be connected with a family health care provider?

Once registered with the program, you will receive an information package directing you to a Care Connector who will work to refer you to a family health care provider. You will be referred to a family health care provider as soon as one becomes available. There is no time guarantee.


How are patients referred to providers?

Priority is given to people with greater health needs. This is decided based on an established clinical process. You will be asked questions about the condition of your health at the time you register for Health Care Connect.


Will the same provider be found for my whole family?

Health Care Connect will make every effort to find the same health care provider to accommodate every member of your family. But there is no guarantee that your whole family will be referred together.


Will I be referred to a family health care provider in my community?

Health Care Connect is designed to refer you to a family health care provider in your local community.


Do I have a say in the health care provider I am referred to?

Once you are referred to a health care provider, it is up to you to make an appointment and decide whether that individual is right for you.

If you choose not to accept the referral, you can return to the program until another provider becomes available, but you will be required to call Health Care Connect and register again.


What if I move?

If you move you must call your Care Connector to provide your new contact information. You will be directed to a Care Connector in your new community who will continue the search for a family health care provider in your new area.


What if my health circumstances change?

Contact your Care Connector if your health needs change. He or she will be able to advise/assist you with next steps.
