Strengthening Home and Community Care

New Vision Puts Patients at the Centre of Care

Ontario is taking action to improve care for patients who receive health care services at home or through community supports.

The province released A Vision for Home and Community Care in Ontario today to achieve better care for patients and better value for Ontarians' health care dollars.

To achieve this vision, the government will hold a series of home and community care summits across Ontario to gather feedback from providers, patients and their families. The summits will focus on patient and caregiver experiences, and share solutions for improving patient care. In addition to the summits, the government will seek input from health care experts on how to improve the quality and value of care provided in the home and community sector.

The government is also planning to move forward with new steps to improve accountability, transparency and quality of care in the home care sector by:

Strengthening home and community care supports the government's Action Plan for Health Care and its commitment to provide the right care, at the right time, in the right place. It is also part of the government's economic plan that is creating jobs for today and tomorrow by focusing on Ontario's greatest strengths – its people and strategic partnerships.
