Connected Care Update

Off to a Great Start!

Thank you to everyone who has applied in the first wave to become an Ontario Health Team. The response has been extremely positive. Over 150 self-assessments have been received from across the province – from as far north as Kenora, as far south as Niagara, and from Lanark County to Chatham-Kent. These applications represent a real commitment from health service providers to work together with the province, to deliver on the vision of an integrated, modernized health care system centred around the patient.

For those of you that have not gone through the self assessment we encourage you to continue your local conversations and planning. The ministry will continue to accept, assess and evaluate groups interested in becoming an Ontario Health Team. The next deadline for applications will be announced in the coming weeks.

Ontario Health Teams are a new model that will integrate care and enable patients, families, communities and providers to better work together. These teams will be responsible for delivering high quality care to their patients, directly connecting individuals and families to the different types of services they need with effective navigation and understanding their community health needs.

Next Steps:

  1. Self-assessment applications are currently being reviewed by the ministry.
  2. The ministry will connect with all applicants in June to provide an update on the status of their application.

The first Ontario Health Team Candidates will be announced in the fall of 2019.
