OHIP Billing Number Registration

Provider Registration

Provider registration is required in order to receive an Ontario Health Insurance Plan (OHIP) billing number to submit claims for insured services.

Health care professionals who are applying for an OHIP billing number to submit claims to the Ministry of Health and Long-Term Care (the ministry) for insured services must:

  1. hold a valid Certificate of Registration with a governing body
  2. have an Ontario practice address
  3. complete and sign the Application for OHIP Billing Number for Health Professionals form
  4. provide banking information to support direct payment (if applicable)
  5. must register for Medical Claims Electronic Data Transfer (MCEDT) and Health Card Validation (HCV) services

In order to complete a registration, please submit:

  1. Completed Application
  2. Banking Information

By one of the following methods:

  • Scanning originals and sending by email:

Pharmacy Registration

A new pharmacy, change in ownership (sale, acquisition), or relocation must register with the Ministry of Health and Long-Term Care (ministry).

To receive a Pharmacy Registration Package, please contact the ministry at:

Pharmacies who are registering with the ministry must:

  • hold an Ontario College of Pharmacists accreditation number
  • have an Ontario practice address
  • complete and sign the Application for Ontario Public Drug Programs Registration form
  • provide banking information (void cheque) to support direct payment (if applicable)
  • register for eHealth Ontario one Mail

In order to complete a registration, please submit only the following documents:

  • Pharmacy Registration Checklist
  • Health Network System (HNS) Subscription Agreement
  • Ontario Drug Program (OPD) Application
  • Void Cheque (Photocopy) or Confirmation from Bank
  • One Mail Application

Completed registration packages can be submitted to the ministry by one of the following methods:
