What you can expect during your MedsCheck appointment

To give the pharmacist an accurate sense of how you take your medications on a daily basis, it might be good to have a few questions in mind before your appointment.

Your MedsCheck Personal Medication Record

The MedsCheck Personal Medication Record is a form signed and dated by you and your pharmacist. It is a record of your medication list including prescription drugs, herbals and over the counter products.  A version of this form will be provided to you for every MedsCheck appointment.

It is important that you or your caregiver bring your MedsCheck Personal Medication Record with you when meeting with your physician or for any scheduled hospital visits. If you have experienced changes to your health and/or medications, or if you’ve recently been discharged from the hospital consider speaking to your pharmacist about a MedsCheck Follow-Up appointment.
