Health Services Appeal and Review Board (HSARB)
If your request for OHIP eligibility, payment for health services or prior approval for out of country medical services were denied by the ministry, you may wish to request a hearing before the Health Services Appeal and Review Board. The procedure of the Appeal Board requires that you mail or deliver a written request for a hearing to the Health Services Appeal and Review Board office within 15 days of receiving your denial letter:- Health Services Appeal and Review Board

The Health Services Appeal and Review Board (HSARB) is established under the Ministry of Health Appeal and Review Boards Act, 1998. Amongst other things it conducts appeals of decisions of the General Manager of the Ontario Health Insurance Plan respecting eligibility for and payment of insured services.
