At this time, information included in Part III (General Benefits / Limited Use) and in Part XI (Registered Nurses in the Extended Health) of the Formulary / CDI can be displayed by selecting one or any combination of the following criteria :

• Generic Name
• Brand Name
• Drug Identification Number/Product Identification Number (DIN/PIN) keyword

Some drug products not listed on the Formulary are reimbursed through the Exceptional Access Program (EAP). Some drug products listed as a Limited Use benefit on the Formulary may be reimbursed through the EAP for other indications. Please see the EAP Reimbursement Criteria.
