(1) Name: The amendments to Section 21 revoke the title “Schedule 5”. OHIP funded physiotherapy facilities are termed “Designated Physiotherapy Clinics”.

(2) Eligibility for service: Insured physiotherapy services are defined as services rendered by designated physiotherapy clinics, when ordered by a physician and rendered by a physiotherapist employed or under contract with a designated physiotherapy clinic to insured individuals who are:

• younger than 20 years of age or 65 years of age or older
• individuals who following discharge as a hospital patient have physiotherapy needs directly connected to a condition, illness or injury for which the individual was admitted to hospital (post acute hospitalization); or
• individuals who require in-home services due to their condition, illness, or injury (including residents of LTC home facilities)
