Changes to Eye Care Services Statement for Optometrists and Physicians Effective November 1, 2004, a new schedule of benefits for optometry services set out the conditions and limits of insured services rendered by optometrists . Amendments were also made to the Physician Schedule of Benefits for the provision of insured eye services rendered by physicians.
