Operational grants :
In May 2003, the Ministry published guidelines for Ontario's Primary Health Care Transition Fund (PHCTF) operational grants application processes. These initiatives focus on interdisciplinary primary health care including research, evaluation, demonstration, mental health and rehabilitation demonstration, accreditation and leadership training projects. There were two deadline dates for applications: June 2003 and November 2003.

In 2002-2003, 15 projects were funded; in 2003-2004, 45 projects were funded; and in 2004-2005, 41 projects were funded. List of all projects

Capital grants :
In June 2003, the Ministry published guidelines for Ontario's PHCTF capital grants application process. These grants support one-time capital costs associated with the integration of primary health care practitioners into interdisciplinary primary care models. The deadline for applications was October 2003.

In 2003-2004, 14 projects were approved for funding and in 2004-2005, 44 projects have been approved. List of all projects

All PHCTF projects receiving Operational Grant funding are to be completed by September 30, 3006 and Capital Grant funding are to be completed by March 31, 2006
