Health Care Provider Access
to Prescription Drug Claims History

Attention Recipients of Ontario Drug Benefit Program

The Ministry of Health and Long-Term Care has been providing prescription drug claims history of patients receiving benefits through the Ontario Drug Benefit program (including Trillium Drug program recipients) to Ontario hospital emergency departments.

This is to notify you that the ministry is now beginning to provide more health care providers with access to the drug claims history of ODB recipients for the purpose of providing health care.

Participating health care providers may only access your drug claims history for the purpose of providing care to you. Health care providers are required by law to protect the privacy of your personal health information.

You have the right to withdraw your consent. Before making a decision to withdraw consent, you are encouraged to consult with a health care provider about potential health risks. A decision to withdraw consent will have no effect on your eligibility for the Ontario Drug Benefit program.

You can change your instructions by submitting a signed consent form to the ministry. Consent can also be temporarily reinstated during your visit to a participating health care provider.

If you have already withdrawn your consent to the sharing of this information with hospital emergency departments, the ministry will not share your drug claims information with any participating health care providers. These instructions will continue to apply unless you request a change.

This information is collected, used and disclosed in accordance with the Personal Health Information Protection Act, 2004.

ODB recipients can obtain consent withdrawal/reinstatement forms from this web site or by contacting the ministry INFOline.
