Examination Application Packages
and Certification Examination Dates

NOTE :

All applications must be post-marked prior to the application deadline.
Late applications will not be accepted.
Only original signed and dated applications will be accepted.
Please do not email or fax completed forms as they will not be accepted.

Applications for people with disabilities are available from ServiceOntario forms.

To complete an application :

You must read the application guide related to the application form prior to completing it.
Ensure that all mandatory fields are completed.
Click the “Print Form” button once you have completed the form. In the electronic form version, the print form button will cause an automatic review of the document and raise an alert if a mandatory field is incomplete.
Sign and date your printed application form. Only signed and dated applications will be accepted.
Ensure the appropriate application fee is enclosed in the form of certified cheque or money order payable to the Minister of Finance. Once received your application will be reviewed.
Submissions are to be mailed to the address listed in the application guide. You cannot submit your application online.

Be sure to review the eligibility section of the appropriate examination information package to determine if you meet the requirements to apply. Click on the type of certification below that you wish to apply for.
