What kind of bone anchored hearing aid (BAHA) replacement sound processor is covered by the Assistive Devices Program (ADP) ?
Any bone-anchored hearing aid (BAHA) replacement sound processor listed with ADP is eligible. An audiologist registered as an ADP BAHA sound processor authorizer can tell you about the makes and models available. Who can apply ?
Anyone with a long-term hearing loss who requires a replacement sound processor for 6 months or longer, and who has not received Ministry of Health and Long-Term Care (MOHLTC) funding towards a sound processor in the past five years, can apply. You must be eligible for Ontario Health Insurance and have a valid Health Card in your name. ADP does not pay for equipment available under the Workplace Safety & Insurance Board, or to Group "A" veterans for their pensioned conditions. What do I do first ?
You start by contacting an audiologist registered as an ADP BAHA sound processor authorizer. If you are eligible for funding assistance, the authorizer will provide you with an ADP Equipment/Supply Authorization (ESA) form. You can also get this form by calling :
Do I fill out the entire ADP form ?
No. You fill in Section one called Biographical Information. What do I do next ?
If you require a replacement sound processor with abutment, you must see a physician who is a hearing specialist (an otolaryngologist). He or she will complete Section 2 of the form. You must also see an audiologist registered as an ADP BAHA sound processor authorizer who will complete Section 3 of the form. If you require a replacement sound processor without abutment, you must see an audiologist registered as an ADP BAHA sound processor authorizer who completes Section 3 of the form. The ADP registered authorizer will write the make and model of the sound processor you need in Section 3 of your ADP form. Who sells me the sound processor ?
You must buy your sound processor from an ADP registered BAHA vendor. The vendor bills ADP for the amount it will pay toward the purchase of the sound processor. You pay the vendor the rest of the cost. How much does ADP pay toward the sound processor ?
ADP pays the vendor 75 per cent of the manufacturer/distributor invoice price in Canadian dollars, up to a maximum of $2625 for the sound processor or 75 per cent up to a maximum of $3366.75 for the sound processor with abutment. For people who are receiving benefits from any one of the following: Assistance to Children with Severe Disabilities; Ontario Disability Support Program; or Ontario Works, at the time the sound processor is authorized, ADP will pay the vendor up to $3500 toward the sound processor or up to $4489 toward the sound processor with abutment. Ask the vendor for more details. Is my sound processor covered by a warranty ?
The sound processor comes with a two-year repair warranty. You are responsible for the cost of repairs when this warranty ends. How often can I replace my sound processor?
ADP will replace your sound processor once every five years if it is not working and can't be repaired at a reasonable cost. ADP will not pay if the original equipment is lost, stolen or damaged due to misuse. You can insure your sound processor under your homeowner insurance policy. | ||||||||||
