What kind of hearing aids are covered by the Assistive Device Program (ADP) ?
Behind-the-ear hearing aids, in-the-ear hearing aids, canal and completely-in-the-canal hearing aids, eyeglass hearing aids and body hearing aids. ADP also covers FM systems and some accessories. What kind of items are not covered ?
You might want to shop around before you decide where to purchase your new hearing aid or FM system. Who can apply ?
Anyone with a long-term hearing loss who requires the use of a hearing aid for six months or more can apply. You must be eligible for Ontario Health Insurance and have a valid Health Card in your name. ADP does not pay for equipment available under the Workplace Safety and Insurance Board. There is a separate fact sheet for adults. What do I do first ?
You start by getting an ADP Hearing Device Application form. You can get this form from an authorizer or vendor registered with ADP, or by calling :
Do I fill in the entire ADP form ?
No. You, your parent or guardian fills in only the first part called Section One. The other parts of the form are completed by your physician, or an audiologist, the ADP authorizer, the ADP dispenser and the ADP registered vendor. Do I need to see a medical doctor ?
First-time applicants and children whose hearing loss is not stabilized must be examined by a medical doctor who is a hearing specialist (an otolaryngologist). The doctor will fill in Section Two of the ADP form to verify your need for a hearing aid. What happens next ?
Your doctor will refer you to an audiologist who is registered with ADP and called an authorizer. If you are already an ADP client, you may go directly to an audiologist registered with ADP. The audiologist will give you a hearing test. He or she will write the exact type of hearing aid, accessories and FM system (if needed) in Section Three of the ADP form. Who pays for the hearing test ?
Sometimes OHIP does. At other times you do. Ask about this before you make an appointment for your hearing test. Who sells me the hearing aid ?
You must buy your hearing aid from a vendor registered with ADP. You will be fitted for the aid by a dispenser who is also registered with ADP. The registered vendor bills ADP for the amount it will pay towards your hearing aid. They then charge you the remaining cost. How much does ADP pay for my hearing aid ?
ADP will pay the registered vendor 75 per cent up to a maximum of $500 of the cost of the hearing aid, earmold, accessories listed with ADP, and the dispensing fee. If you need hearing aids for both ears, the vendor will bill ADP for 75 per cent up to maximum of $1,000 of the cost of two hearing aids, two earmolds, accessories listed with ADP, and two dispensing fees. The earmold must be bought at the same time as the hearing aid. ADP will not contribute to the cost of earmolds bought separately from hearing aids. The registered vendor will charge a dispensing fee to fit the hearing aid. The dispensing fee may vary depending on the type of hearing aid. For FM systems, ADP will pay the registered vendor 75 per cent up to a maximum of $1,350 of the cost of the ADP listed device and dispensing fee. Is there a trial period for me to try out my hearing aid ?
Yes. You have 30 days to try out your hearing aid. If you are not completely satisfied with it, you should return it to the vendor before this trial period ends. The vendor may charge you a handling fee. Is my hearing aid covered by a warranty ?
Yes. A manufacturer's minimum one-year warranty comes with every hearing aid. However, once the warranty expires, you are responsible for the cost of repairs. Can I replace my hearing aid if my medical condition or hearing changes ?
If your authorizer confirms that there has been a significant change in your medical condition or hearing, he or she can contact ADP and request assistance for a replacement hearing aid. How often can I replace my hearing aid if my medical condition does not change ?
If your hearing aid is no longer working, and cannot be repaired at a reasonable cost, ADP will help replace it once every three years. If your FM system is no longer working and cannot be repaired at a reasonable cost, ADP will help replace it once every three years. ADP does not pay for replacement during those years if the original equipment is lost, stolen or damaged due to misuse. You may want to insure your hearing aid. | ||||||||||
