Printable version

What kind of orthotic devices are covered through the Assistive Devices Program (ADP) ?
  • specified custom-made braces and splints
  • selected paediatric orthoses
What kind of orthotic devices are not covered through ADP ?
  • braces for less than 6 months use
  • braces worn only at night or to rest a body part
  • braces worn for only one daily activity, such as work, school or sports
  • braces for acute conditions
  • foot orthoses
  • shoes and shoe modifications
  • prefabricated orthoses
  • centrally fabricated orthoses
  • treatment machines
Who can apply ?

Any resident of Ontario who has a valid Health Card and a long-term, physical disability requiring an orthosis for 6 or more months to perform a variety of daily activities.

ADP does not pay for equipment available under the Workplace Safety and Insurance Board or to Group 'A' veterans for their pensioned conditions.

What do I do first ?

You begin by getting an ADP Equipment/Supply Authorization (ESA) form. You can get this form by calling one of these numbers :

Toronto :   416-327-8804
Toll Free :   1-800-268-6021
TTD/TTY :   416-327-4282
TTD/TTY Toll Free :   1-800-387-5559

You can also get this form by visiting a vendor who is registered with ADP. Call one of the above numbers for the names and telephone numbers of vendors in your area.

Do I fill in the entire form ?

No. You must fill in the first section called Biographical Information.

Do I need a medical checkup ?

Yes. All applicants must be examined by either a general practitioner or a specialist physician, depending on the complexity of the device. If required, the specialist must be a geriatrician, neurologist, neurosurgeon, orthopaedic surgeon, paediatrician, plastic surgeon, physiatrist, general surgeon, rheumatologist, radiation oncologist or medical oncologist.

The physician will fill in the information about your disability in the second section of the ADP form.

What happens next ?

For orthotic devices, your general practitioner or specialist will refer you to an authorizer approved by ADP. These authorizers are Canadian board-certified orthotists.

The authorizer will determine the exact type of orthotic device you need. He or she will then fill in Section 3 of your ESA form.

All clients with a chronic, complex disability must see both an ADP authorizer and a rehabilitation assessor. Rehabilitation assessors are occupational therapists or physiotherapists registered with ADP.

The rehabilitation assessor will determine if you need physiotherapy or occupational therapy and will also sign Section 3 of the ESA form.

Where can I get my orthotic device ?

From any vendor in Ontario who is registered with ADP. The vendor will bill ADP directly for a portion of the cost of the orthosis.

How much will ADP pay ?

The vendor will bill ADP 75 percent of the approved cost of your device. You will be charged for the remaining 25 percent.

If you are receiving social assistance benefits under Ontario Works (OW), Ontario Disability Support Program (ODSP) or Assistance to Children with Severe Disabilities (ACSD), you will be eligible to receive additional funding to 100 percent of the ADP approved price.

Will my health insurance company cover my share of the cost ?

Many private insurance plans will share your cost. If you have private medical coverage, check with your insurer or agent to see if they will help pay for your share of the cost.

What if my medical condition or size changes ?

ADP will pay up to 75 percent of the replacement cost if your medical condition or size changes and you need a new orthotic device.

What if I need to replace my equipment for some other reason ?

ADP will pay a portion of the approved amount if your orthotic device is worn out due to regular use.

ADP does not pay for replacement if the original equipment is lost, stolen, or damaged due to misuse. You are encouraged to buy insurance for coverage in these situations.
