Organ and Tissue Donation

This fact sheet provides basic information only. It must not take the place of medical advice, diagnosis or treatment. Always talk to a healthcare professional about any health concerns you have, and before you make any changes to your diet, lifestyle or treatment.

If you are 16 years of age or over, you can resister your consent to donate your organs and/or tissues upon your death. Complete a Gift of Life Consent Form (3750-84) and submit it to the Ministry of Health and Long-Term Care who keeps a registry of donors.

You can indicate your consent to donate your organs and/or tissue for transplant, or for transplant and research. This is not the same as giving your whole body to science or to a medical school. Your body will still be available and presentable for a funeral.

After completing and signing the form (please use white paper only), return it to :

You can change or withdraw your consent at any time by writing to the address above.

If, when registering your consent to donate:

  • you carry a photo Health Card, you will be sent a new card. It will be the same as your previous card but will have:
    • A new “card version code” (two letters printed beside your Health Number to show that this is now the current card);
    • The words :”donor/donneur” on the back of the card
    • A code on the back of the card representing your choice.
  • you carry a red and white Health Card, you will not receive a new card. You will be sent a notice confirming that the data has been updated. You will also be given a sticker with the words “doner/donneur” to put on the back of your Health Card.

You can register your consent at any time – while registering for health coverage (for the first time, getting a new card (if you have lost or damaged your card), renewing your card, or changing information on your Health Card or even if you have no other business with the ministry.
