Organization: Yee Hong Centre for Geriatric Care Project: Culturally and Linguistically Appropriate Day Program for Adult and Senior Stroke Survivors in the Chinese Community Contact: Maria Chu Yee Hong Centre provides a culturally and linguistically appropriate day program for adults and seniors in the Chinese community. It offers services to support post-stroke, frail and dementia individuals to maintain and enhance their abilities, live and function independently in the community, and provide information and respite to their families. The program includes activities such as meals, music, current events, education, health monitoring and occupational therapy. This program is innovative because it fills the existing gap by providing a continuum of care services. Yee Hong Centre works closely with multidisciplinary health care professionals to help discharged Chinese stroke survivors access their services. It is also innovative because of the culturally and linguistically appropriate integrated care.
