Nurse Practitioner-Led Clinics
25 Nurse Practitioner-Led Clinics (NPLCs) were announced in 2007 as part of the Family Health Care for All Strategy and are targeted to be fully operational by 2012. This new model of care will see nurse practitioners working in collaboration with doctors and other interdisciplinary health care providers delivery health care to many Ontarians who previously did not have access to a primary health care provider. The Clinics will not only focus on providing better primary care to patients, but through education will work with patients about disease prevention, health promotion and improved self care in management of chronic diseases. The Clinics will help patients navigate the health system – connecting them, when needed, with other service providers and with community-based programs and services. Call for Applications – Wave 1
