Ontario Health Teams
The government is building a connected health care system to improve the patient, family and caregiver experience. These changes will make it easier for them to navigate the system. Having health care providers work more closely together will take the guesswork out of transitions, where we know patients often feel lost and unsupported.
Ontario Health Teams are being introduced to provide a new way of organizing and delivering services in local communities. Under Ontario Health Teams, the health care providers (including hospitals, doctors and home and community care providers) will work as one coordinated team – no matter where they provide care. These teams will support patient care where and when they need it, and help them transition from one health care provider to another seamlessly.
The Ministry of Health and Long-Term Care has provided resources to support health care providers and organizations to become an Ontario Health Team.
Becoming an Ontario Health Team
Changing the way care is delivered will take time and dedication from all system partners.
Below are some important dates for Ontario Health Teams applicants:
April 3, 2019 — Open call for self-assessments
May 15, 2019 — Deadline to submit self-assessments
June 3, 2019 — Selected groups will be invited to submit a full application
July 12, 2019 — Deadline to submit full applications
Fall 2019 — Announce Ontario Health Team early adopters
Future deadlines will be announced — The ministry will continue to accept, assess and evaluate groups interested in becoming an Ontario Health Team.
Throughout this process, the Ministry of Health and Long-Term Care will continue to accept, assess and evaluate groups who are interested in becoming an Ontario Health Team.
