Memorandum – Vendor Registration for Home Oxygen Therapy

The Ministry has issued an invitation to establish a list of qualified vendors for home oxygen therapy. Beginning April 1, 2017, only applicants who respond to the invitation and are approved by the Ministry will be:

Read the Guide to Apply for Registration as a Vendor for Home Oxygen Therapy before you begin to complete the application form, as it contains essential information about the requirements for registration with ADP as a vendor for home oxygen therapy.

On page 23 of the guide, you will find a checklist of documents that are required as part of your application for registration. Use this list to ensure that you are including all of the required documents with your application, including the Confirmation of Payment Instructions form.

Manuals

Read the Policies and Procedures Manual for the Assistive Devices Program and the Policy and Administration Manual – Home Oxygen Therapy before submitting an application for registration as a vendor for home oxygen therapy. The manuals, together with the Vendor Agreement form every vendor’s contractual agreement with ADP.

The template for the Vendor Agreement is available for your review so that all prospective vendors will have an opportunity to read the contract that they will be required to sign. Do NOT complete and sign this template. If approved as an ADP registered vendor for home oxygen therapy, the ADP will send out a completed agreement to you.

Important Dates

September 14, 2016: Ministry of Health and Long-Term Care issued an invitation to interested parties to become an ADP registered vendor for home oxygen therapy.

September 28, 2016: ADP will answer all questions submitted on or before this date. Please send all questions to adp@ontario.ca.

October 13, 2016: ADP will provide questions and answers to any person or entity on the notification list.

October 19, 2016: To be considered for registration as a vendor for home oxygen therapy, ADP must receive the application by 4:00 p.m. on October 19, 2016.

Submitting an Application for Registration as a Vendor for Home Oxygen Therapy

ADP requires original signatures. The completed application must be delivered by mail, courier or in person to:

Applications will not be accepted by fax or e-mail. Applications will not be accepted after the deadline for submission. ADP is not responsible for applications not received, or received late, due to error by the applicant or a delivery agent.
