2014 Minister's Medal winners

The Ministry of Health and Long-Term Care is proud to showcase the inaugural winners of the 2014 Minister's Medal in both the Team and Individual Champion categories:

Team Recipient : Toronto Central Integrated Palliative Care Program [PDF]

The Toronto Central Integrated Palliative Care Program is based on a partnership formed between the Temmy Latner Centre for Palliative Care and Toronto Central Community Care Access Centre (CCAC). The program creates one interdisciplinary, integrated care team with the goal of providing individuals with life-limiting illness and their families with high-quality, 24/7 client-centred care, enabling client choice, easing pain and suffering and optimizing comfort, dignity and quality of life. Using no new net dollars but putting existing resources to better use, the program has demonstrated significantly higher quality and value, including strong client satisfaction and significant savings to the system.

Individual Champion Recipient : Dr. Linda Lee, Centre for Family Medicine Memory Clinic [PDF]

Working in primary care for 26 years, Dr. Lee noticed that patients with cognitive impairment were not getting the best possible care. Identifying this need, she developed a comprehensive multi-disciplinary program, using evidence-based tools for diagnosis of cognitive dysfunction and best practices in patient care to provide accurate, efficient diagnosis and holistic care for patients and their families. After empowering those within her own family health team, she developed a comprehensive training course in which she instructs and mentors others which has led to the creation of 56 other memory clinics throughout Ontario to date.
