Disputing a Claim

Medical records are held by health care professionals and contain detailed medical history information. Medical and clinical records are not sent to the Ministry of Health. Under the Health Insurance Act, health care professionals and clinics are responsible for retaining medical records. A Personal Claims History (PCH) is a computer record of claims paid by the Ontario Health Insurance Plan (OHIP), under an individual health card number.

If an individual or their substitute decision maker disagrees with information contained on the personal claims history, they may contact the Ministry of Health at 1-800-262-6524. Disputing a claim will not remove the claim from the ministry’s computer record but it will flag the claim as having been disputed.
