Definitions of headings on a Full Personal Claims History report:

Health No: The unique 10-digit Ontario health card number assigned to the individual whose PCH is being requested

Name: The individual’s first and last name.

Service Date: The date service(s) were received.

FSC (Fee Schedule Code): A code used by the ministry to identify the service provided. For a more detailed description of each FSC, please consult the Schedule of Benefits.

FSC Description: A brief description of the service provided.

Clinic No: If you received services from a clinic, the ministry uses this unique number to identify which clinic was visited.

PCN (Primary Care Network) Clinic Code: This code is used to determine what type of clinic provided the service(s).

Provider: The ministry uses this unique number to identify which health care professional provided the service.

Spec Cd (Specialization Code): If the service was provided by a specialist, the ministry uses this code to identify the type of specialist that provided the service(s).

Refer Phys N No: The ministry uses this unique number if the service resulted from a referral from another health care professional.

# / Serv: The number of units billed for this visit.

Fee Paid: The fee paid to the health care professional for the service.

Assess or Run Date: The date the claim was processed by the Ministry of Health.

Hospital No: If you received services from a hospital, the ministry uses this code to identify the specific hospital that provided the service.

Explan: Explanatory code used by the ministry to process payments to the health care professionals.

PTP: Payment institution type code used to process payments.
